Reopened Claim Stamps for Insurance File Continuity

A closed insurance claim may return because of new documentation, supplemental damage, a payment issue, recovery activity, a complaint, litigation, or an internal correction. Reopening the file does not erase the first investigation or automatically change the prior decision.

A controlled stamp set should connect the reopening event to the original claim, identify the new evidence, and show which reviews must be repeated. It should not make coverage, liability, valuation, legal, or payment decisions outside the authorized process.

The claim history should retain:

  • claim reference;
  • original loss or event reference;
  • prior closure date and reason code;
  • last payment or denial reference;
  • outstanding recovery or litigation status;
  • retention or archive location.

Do not remove the CLOSED mark. Add a new REOPENED event with its own date, trigger, source, and owner.

Use REOPENING REQUEST RECEIVED until an authorized person confirms the file should reopen.

The covers first notice and initial escalation; the reopened file needs an explicit bridge to that original history.

A reopened file often contains a mixture of prior and new material. Mark each addition with received time, source, document type, version, page count, and relationship to the trigger.

Useful states include:

  • NEW EVIDENCE RECEIVED;
  • DUPLICATE OF PRIOR RECORD;
  • SOURCE VERIFICATION PENDING;
  • MATERIAL CHANGE REVIEW;
  • PRIOR DECISION REVIEW REOPENED;
  • DOCUMENT INDEX UPDATED.

Do not restamp old evidence as newly received. That destroys chronology.

A property claim closed after an initial payment. Three weeks later, a contractor submits a revised estimate. The intake record might show:

This wording prevents the new document from being mistaken for approval of the additional amount.

A new invoice may require payment review but not a fresh coverage decision. New damage information may require several reviews. State the scope:

  • COVERAGE REVIEW REOPENED;
  • LIABILITY REVIEW REOPENED;
  • DAMAGE / VALUATION REVIEW;
  • RESERVE REVIEW;
  • PAYMENT AUTHORIZATION REVIEW;
  • RECOVERY REVIEW;
  • CUSTOMER COMMUNICATION REQUIRED.

A broad “FULL REVIEW” label should be used only when the organization has defined what that entails.

Reserve changes, payment calculations, authorization, issue, delivery, return, and reconciliation are distinct. Use marks that point to the controlled financial record.

A clear sequence can be:

  1. RESERVE REVIEW REQUESTED.
  2. RESERVE ACTION RECORDED.
  3. PAYMENT CALCULATION CHECKED.
  4. AUTHORIZATION PENDING.
  5. PAYMENT ISSUED.
  6. DELIVERY / RETURN EXCEPTION.
  7. RECONCILED.

Avoid putting amounts on widely circulated covers when a secure transaction reference is sufficient.

A phone attempt is not completed communication. Record channel, date, recipient role, message or notice version, result, and next action.

Useful states include CONTACT ATTEMPTED, DOCUMENT REQUEST SENT, RESPONSE RECEIVED, NOTICE ISSUED, DELIVERY CONFIRMED, and COMMUNICATION EXCEPTION. The policy and jurisdiction determine required content and timing.

Before reclosure, confirm what changed from the prior closure:

  • new evidence reviewed;
  • decisions repeated or left unchanged;
  • payment or recovery action;
  • complaint or legal state;
  • customer communication;
  • remaining obligations;
  • final owner and date.

Use RECLOSED — SUPPLEMENT RESOLVED or another specific reason rather than simply applying CLOSED again.

The file is continuous only when the second period can be understood without rewriting the first.
